For and in consideration of participating in the UT7 Football organized events, I hereby release, acquit, forever discharge, indemnify and hold harmless UT7 & X-Roads Warriors FC organizers including all coaches, sponsors, operators, facilities used and their respective officers, directors, employees, volunteers, agents and representatives of and from any and all claims, demands, actions, or causes of action, liabilities, or damages arising out of or in any way related to my participation in the UT7 Football Camp and any related activities. I further certify that I have no known or existing medical conditions that prevent me from participating in physical exercise and activities fully. In the event if injury or illness I hereby grant authority to be qualified physician to render such medical treatment as said physician deems reasonable and necessary, and we also accept responsibility for any expenses involved with such illness, injury and treatment.
