For and in consideration of participating in the UT7 Football Series organized events, I hereby release, acquit, forever discharge, indemnify and hold harmless UT7 Football Series organizers including all coaches, sponsors, operators, facilities used and their respective officers, directors, employees, volunteers, agents and representatives of and from any and all claims, demands, actions, or causes of action, liabilities, or damages arising out of or in any way related to my participation in the UT7 Football Series and any related activities. I further certify that I have no known or existing medical conditions that prevent me from participating in physical exercise and activities fully. In the event if injury or illness I hereby grant authority to be qualified physician to render such medical treatment as said physician deems reasonable and necessary, and we also accept responsibility for any expenses involved with such illness, injury and treatment. 
Participation includes possible exposure to illness from infectious diseases including but not limited to COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation; and, I willingly agree to comply with the terms for participation in regards to protection against infectious diseases. I also hereby give permission for UT7 to use images/videos of me for promotional materials on Facebook or any other social media Platforms.
